天气变冷,颈肩痛的病人多了起来。特别是那些需要伏案低头读书,频繁使用电脑手机,以及压力大晚睡的人。有的是颈项部,有的是肩胛骨到脊柱之间,有的是颈部到肩部之间。这些肌群其实联系紧密,在运动颈肩部的时候互相牵制,一个肌肉受损,因为作用力的缘故,很容易影响到其他肌肉。通常是,其中一个肌肉好点了,旁边的肌肉又开始不舒服了。有的是有颈部向后背部扩散,有的是从颈部向肩部扩散。
中医伤寒论有云“项背强几几”(读作“殊殊(shū shū)”) 是太阳经病。足太阳膀胱经主之。中医讲整体治疗,通常全身包括手脚都选取穴位治疗。可是不懂中医的人就不接受,其它部位不疼,为什么要扎,并且还怕被扎坏了。其实并不是这样,手脚是最不怕扎坏的,如果选穴手脚又有效,其实是风险最小的。整体治疗的思路就是打通经络,气血才从全身运行顺畅,从而聚集到局部起到修复作用。但是要病人接受还是需要时间的。而且医疗保险这块也是比较烦,如果病人投诉说你乱扎你就有口说不清。所以基本刚开始肯定是局部选取穴位治疗了。即使是这样效果还是很好的。
扎完疼痛就轻微了很多。有的病人告诉我,早知道针灸这么好,早就应该来了。说看了整骨医生,整的时候轻松了,可是一出诊所就立马恢复原样,而且去了好多次都这样。针灸就不一样,虽然效果慢些,至少能维持一段时间,而且不怕整骨师卡卡给弄坏了。俺也不知道,但针灸师也是要极其小心的,不能给人扎位置不对,而且颈肩部穴位相对手脚穴位还是更有风险的。不过诊所的这些个病人针后反馈都很好,对我连声感谢的,我也很高兴。

针灸能够止痛的现代医学原理是, 针灸可以刺激中枢神经制造和释放 Endolphen, Enkaphalin 等这些内源性的类阿片神经介质, 起到暂时止痛的作用。 一旦这些物质消失了, 疼痛就会复发。
针刺效果好不好看人,相当多的案例中是可以做到永久性解除疼痛的。这个永久是指6+个月不服用药物也不做外部治疗而保持无痛状态,不良的习惯和缺乏运动的生活状态会让身体结构重新歪斜再次疼痛那就是另外的故事了。
我相信目前科学研究不支持任何神经介质一次分泌可以持续如此长的时间有效。
而整脊师(chiropractors)是所有疼痛治疗中我唯一非常不推荐病人去看的项目,这个治疗多数情况下对病情缓解无效不说,相当程度上还有可能会让病情恶化,关于这一点也可以询问MD,他们对整脊治疗更为了解,也颇有微词。
