请问大神这个表格应该怎样填

请教大神们，现在的公司还没有叫我回去上班。但申请失业时应该怎样填写才可以继续拿到失业金？我们这个州准备要复工50%，但每天疫情感染病例还是持续上升，家里又有小孩确实不想那么快上班。
