实际操作起来，小分子化学药的可行性还大一些。大分子药（Biologic) 操作起来会很难。化学药的仿制品叫 generic, Biologic的仿制药叫 Biosimilar, 到目前为止还没有听说有很成熟有效的 biosimilar 取得很大成功。
举个例子，Abbvie的摇钱树Humira，专利过期好几年了，市场上也有很多厂商尝试仿制。Abbvie从这一个药已经有2000亿美元的营收，已经经营了20年。但是2022年他家的销售额还继续小幅增长，没有被仿制药影响多少啊。

新药研发路上的风险太高了，成本也太高了。政府应该做的是，想办法降低研发成本。 研发成功之后还要被政府砍一刀，谁还做研发啊？ 现在的趋势已经不太好了，由于高昂的成本和巨大的风险，大药厂已经在慢慢放弃研发， 准备收购成功或快成功的药。现在很多小药厂也岌岌可危。再这么搞搞，走研发的小公司也很难生存了

你要知道不少药厂有一个大部门叫market access，专门对接各种保险公司的。药厂为了能在保险公司的coverage竞标时获得较高的优先级，会offer给保险公司很高比例的rebate。有些条件每一个处方的rebate甚至会超过药品标价的50%！ 有时候你从账单上看以为是保险公司付了不少，自己只付了药品原价的一部分，其实说不定保险公司还从rebate里倒赚了。就这样还有保险公司虚报冒领rebate

你不能拿covid的疫苗来说事。这次是紧急公共卫生事件 政府有拨款是为了投入最大人力物力把疫苗快点弄出来。这个没有代表性。
实际情况是，一款药物研发平近十年才能开始卖钱。这期间投入无数研发成本，但这其实不是主要的。主要的是另外几十款失败的药物的沉没成本。必须靠着成功的这一款赚钱，才能持续药物研发。拜登这个白痴是要自毁武功。
迄今为止，现在市面上的大药厂，没有一个药是美国纳税人的钱来cover 从研发到审批投产的全部费用，甚至主要费用的
现在有些人argue的是， 很多药在研发阶段，是在大学，non-profit机构等利用国家经费研发出来药物（或者前体，lead comound等），比较常见的就是NIH经费。 以及药厂的药物进入国家和州的药物采购，比如medicare ，medicaid 等订购 这些说法基本不成立，因为大学等研发出来的药物，都是通过商业合同license转移给药企进行继续研发的， 药企是付了钱的。 而且后面花费的大头，是development和clinical trial。 这些都是药企自己筹集资金进行的。 而至于采购药物，实际上是药企完全自担风险，已经成功之后的商业采购行为， 政府并没有承担风险。
最近的新冠期间有国家对疫苗研发的支持，这属于危机管理， 并不是常态。
上面说的可能有特殊例外，但是大体如此，所以所谓 （少量）“ 花纳税人的钱”，并不能剥夺药企定价/盈利的权利。 这个从道理上说不通， 从法律上更说不通。
个人同样反对药家高涨，但是要知道药价高的原因是药物研发的成本高 （药企的利润率在业界科算不上高的） 严格监管下的药物研， 发， 生产， 本身成本就极高。 clinical trial 更是天价， 而生产出来之后应对层出不穷的天价诉讼更是要反复大出血 -- 不仅患者，还有各州政府都把药企当肥羊来宰。想让药价降下来， 必须这些环节都降价， 你不解决这些问题，光让药企降价， 药企喝西北风么？ 谁还研发？ 甚至如果生产销售就亏损，人家连继续生产都会停掉。 这就好像银行不许房东停止交房贷，国家却允许租客不用付房租一样可笑。
中国药品集采，是20年医改少有的成功之处。集采前，药品首先考虑是利益空间，卖得好的都是无效高价的辅助药。集采导致企业互相竞争，不管国企外企都卷到地板价。自然利益输送空间也没有了。倒逼企业研发新药，因为新药专利独家，可避免价格厮杀。也可看到15年以后，国内新药研发发展非常快。近几年国内研发的新药陆续上市。
