老美的COMMENT： The barefoot doctors system has died out in China but is emerging in the U.S. and becoming more and more pervasive. U.S. medicine is no longer controlled by the patients or by the doctors (who are relegated now to be called just "providers"). Rather, it is controlled by the 3rd party payers (Medicare, Medicaid, HMO''s, PPOs) who alone holds the purse string. They have the power to shape American medicine to THEIR benefits and they are doing exactly that. Cost control means doctor control, or more specifically, SPECIALIST CONTROL.
