今天有两项十分重要的新型冠状病毒SARS-CoV-2两种关键突变株(英国/南非)的研究原始数据被公布:1. 剑桥大学Gupta实验室在推特展示了接种辉瑞/BioNTech的COVID-19疫苗BNT162b2对英国突变株B.1.1.7保护力变化的原始数据;2. 南非突变株的501.V2科学会议视频被上传到了YouTube,链接可见评论。
根据最新数据(图1),全球已有57个国家发现了英国B.1.1.7突变株;13个国家发现了南非501Y.V2突变株(即B.1.351株)。这两种突变株的输入性传播均在中国被检出。但从今天公布的数据看来,南非的501.V2毒株十分值得警惕。而我之前总结过的包括B.1.1.7和501.V2等含N501Y突变的毒株的一些体外实验原始数据可见以前的微博(O子陵在听歌)。
1. 截止到目前,Fred Hutch研究所的Bedford分析的6089个丹麦SARS-CoV-2病毒基因组序列中,3%为英国B.1.1.7突变株;3093个美国病毒基因组序列中,1%为B.1.1.7突变株。B.1.1.7所占比例增长十分迅速。今天剑桥大学教授、传染病医生及HIV耐药突变专家Ravindra Gupta在推特上传了他一个重要的实验结果。
Gutpa这项研究构建了B.1.1.7的假病毒,这个假病毒含有del69/70,del144/145,N501Y,A570D,P681H,T716I,S982A,D1118H突变。该研究选取了15名接受BNT162b2首次接种后21天的接种者血清,发现相比野生型毒株,这些接种者的血清对B.1.1.7的中和活性(ID50)显著下降,下降中位数为3.85倍(IQR 2.68-5.28)。虽然接种者血清对B.1.1.7中和活性下降,但接种者血清依然对该毒株具有中和活性(图2)。Gutpa认为del144/145突变可能对中和活性影响较大。之前强调过的,相比PRNT50/80,假病毒中和试验具有一定缺陷,因此现在还需要大样本量的疫苗加强接种7天以后的金标准中和试验进行进一步验证。
2. B.1.1.7虽然棘手,但从目前证据来看,南非毒突变株501Y.V2的E484(位于RBD内)突变对于既往感染和疫苗诱导的中和抗体的抵抗性更显著(O子陵在听歌)。华盛顿大学Bloom实验室之前上传的数据证明3/8的COVID-19康复者血清对E484K突变株的结合(或中和)活性降低10倍或以上(O子陵在听歌)。
Bloom组在今天南非突变株的501.V2的科学会议上进一步报道了随访研究,发现11个COVID-19康复患者中9个的血清样品对含E484突变的毒株的中和活性显著下降(82%),其中部分样本下降10倍以上。而来自南非国家传染病研究所(NICD)的研究结果更让人忧虑,该研究发现44个COVID-19康复患者中的21个的血清(48%)对501.V2突变株的完全失去了结合能力,即病毒表位发生了完全逃逸(图3)。这符合我们之前预测的病毒发生了抗原漂移(O子陵在听歌)。但幸运的是,T细胞对501.V2突变株的免疫反应依然保存,这可能在一定程度可以帮助宿主预防重复感染。
这两项研究结果分别提示,虽然COVID-19疫苗接种者血清对B.1.1.7假病毒中和活性下降,但依然对其具有一定中和活性;而COVID-19康复者血清对501.V2中和活性显著下降,在小型研究中48%的康复者血清对501.V2完全失去结合能力(病毒完全逃逸),因此501.V2可能发生了较完整的抗原漂移,这为全球抗疫带来了更大的挑战:追加疫苗接种有可能会成为现实,因此疫苗生产企业需要及早筹备针对新突变株疫苗的生产和投放。

我个人的观点是这项研究进一步说明疫苗诱导的T细胞免疫至关重要,只有抗体没有T细胞免疫的疫苗碰到突变毒株以后有效率会大打折扣。另外很可能明年的这个时候疫苗需要重新设计,每年秋冬季节打一针covid shot将会成为新常态。

看来是病毒关键蛋白结合位点变异了,导致以前的抗体无法起作用。只能指望T细胞免疫记忆。简单说,大统领再打8g蛋白,可能也根本不管用。
如果研究定论了,感觉这个南非病毒可能会诱发ADE吧。

看了一下报道,说“新病毒会让一小半6个月前康复者的血清失效”,那不就是说还有一大半有效吗?再说同样的抗体对同样的病毒,要么都有效要么都无效,一小半失效是什么鬼? 而且我记得康复者抗体本来也就只能存在六个月,这个实验看来只能说明一小半康复者在6个月后没有抗体了而已。

这个Peter Daszak 很可疑,他一会说不知道RaTG13, 一会说放冰箱里就给忘了7年,直到去年疫情爆发才想起来,结果又被人挖出来他们早就拿蝙蝠病毒做gain-of-function实验。他还是中国坚持的世卫专家组成员调查病毒起源 (可想而知病毒起源能调查出什么结果了)。

这是不是就是前些天一直在吵的,现在疫苗对南非变种效果大打折扣?现在就开始研究新疫苗吧。看现在这速度,上半年健康人也不见得能打上,干脆安心下半年打新疫苗,希望到时候能优化疫苗,副作用小点。

这个Peter Daszak 很可疑,他一会说不知道RaTG13, 一会说放冰箱里就给忘了7年,直到去年疫情爆发才想起来,结果又被人挖出来他们早就拿蝙蝠病毒做gain-of-function实验。他还是中国坚持的世卫专家组成员调查病毒起源 (可想而知病毒起源能调查出什么结果了)。
