My wife delivered our second son on July 5th. Everything went smoothly till the day before yesterday. On July 12th, my wife passed a piece of tissue of an egg-size, pictures attached. We contacted on-call OB doctor, who suggested us to contact our own OB nurse the next day. We called our OB nurse the second morning, 13th, and was told to go to ER immediately. The ER doctor ordered ultrasound, which detected abnormality of the uterus. The on-call OB and our own OB both came to look at the results and decided to go for D&C (Dilation and curettage), which was done in the afternoon. Our own OB performed the operation and cleared several more pieces of tissues.
The concern we have is that:
The tissue that my wife passed on 12nd is quite big, about egg-size. It should not be missed easily if the OB checked the integrity of the placenta carefully. (Correct me if I am wrong.) Based on my observation, the resident OB who delivered our second son on July 5th (not our own OB) did not check the placenta carefully after delivery. She just took a briefly look at the placenta, even without touching it in the container. I remember two years ago, when our first son was born, our own OB flipped the placenta back and forth to make sure everything was there.
We kept the tissue sample in 4oC till going to the ER. It was sent to pathology for analysis. We don't know about the results yet.
Any idea, opinion, suggestions are welcome! We would like to file a report to the hospital. Any one has experience before? Thank you all!

在D&C以后,妈妈现在已经恢复的很好了 (15日)。出血已经慢慢停了。精神好了很多。谢谢大家的关心和支持。
虽然我们自己也还是不确定时不时应该跟医院投诉,但是至少,我们会file一个report到医院的相关部门。得到结果以后我们会尽量发个贴给大家参考。
